DailyDoc is a collaborative and continuous care platform that helps you achieve better outcomes in your diabetes care. With DailyDoc, you work with your care team on shared goals and care plans. DailyDoc enables you to monitor and track your health data on a daily basis. It also helps you achieve better medication adherence through medicine reminders and records your insulin dosages. Through clinician dashboard, your care team is able to review your progress and can optimize your treatment remotely on the cloud synced platform. Also you can communicate with your health coach and doctor through secure messaging, directly from your DailyDoc app. Your health coach works with you for delivery of our proprietary programs that enable you make sustainable behavioral changes. If your care team is not on DailyDoc, you can use it as for self-management and send your data and reports to your doctor directly from the app.

# Logging – Log and monitor parameters relevant in your diabetes care like blood glucose, carbs, physical activity, HbA1c, weight, waist, blood pressure etc and tag them with their descriptions like pre-meal, post-meal, breakfast etc. DailyDoc is suitable for all diabetes types including Type 1, Type 2 and Gestational diabetes.

# Diabetes Health Goals – Configure your shared health goals related to your blood glucose levels, HbA1c, carbs intake, daily physical activity and see them bench-marked with your current data on your daily app screen.

# Coaching and support – Your health coach works with you to create detailed action plans to help you achieve health goals. Your coach also monitors your progress and educates you with a diabetes related information and choices that can help you become an expert at managing your diabetes.

# History – See all your data logged in a day-wise and month-wise format. Delete or make changes to any erroneous readings that you might have entered.

# Analysis – All the data relevant to your diabetes care is arranged in the form insightful graphs that can help you identify your trends and triggers. See your averages, highs, lows, co-relation between different parameters and identify your areas of improvement.

# Reports – If your doctor is not on DailyDoc you can generate summarized PDF reports of your data and directly email them to your doctor to enable him take the best therapeutic decisions for you. You can also take export of all your data in the form of well sorted excel sheets

# Medication adherence – Medication non adherence is costly and directly impacts your diabetes results. DailyDoc lets you create your daily medication schedule including your oral meds and insulin dosages and gives reminders to you to take them at the prescribed timings. Your medication data is also helpful in knowing your medication adherence rate and your areas of improvement
